REFLECTIONS ON THE FATAL APOLLO 1 FIRE. Apollo 11 Launch Operations Checklist, page 5-1 / 5 -2, a single sheet in red tint to assist quick location during emergencies. Printed recto and verso. NASA/MSC, April 15, 1969, revised June 1, 1969. 8 x 5½ inches. With a Typed Letter Signed by BUZZ ALDRIN. BUZZ ALDRIN'S provenance letter reads in part: "Accompanying this letter is a page numbered 5-1 and 5-2 from the CSM 107 (Apollo 11) Launch Operations Checklist, SKB32100080-306. The pages are part of the complete manual that was used in the Command Module simulator ... The sheet is from Section 5 titled: Launch Emergency Procedures. "Side 5-1 has the procedure to quickly open the Side Entry Hatch while still on the launch pad or after an emergency landing. Other steps on this side include the abort steps in case of fire in the spacecraft during launch. Those steps include the correct separation method for the Saturn V booster based on time from lift-off. The final steps on this page are the procedures to don our oxygen masks and / or completely don our PGA's (Pressure Garment Assembly or space suits). Side 5-2 has the remaining steps for this equipment donning and methods to solve contamination in our suit oxygen loops. The last steps on this page on this page are procedures to follow in case of a Launch Escape System failure. A fire in the spacecraft is an extremely dangerous occurrence because Neil, Mike, and I clearly remember the tragic loss of fellow astronauts Gus Grissom, Ed White, and Roger Chaffee due to such an event while on the launch pad back in January 1967...." INSCRIBED and SIGNED by BUZZ ALDRIN: "Used in training for Apollo XI, Buzz Aldrin" on page 5-1.
