The document, admission form to the AMUBTW, two identical portions separated by a vertical perforation with each portion signed by him and additionally signed by Lane, accomplished in red ink in an unknown hand. 2 pages, 8vo, written on first and terminal pages; faint scattered foxing. The check, to the AMUBTW in the amount of 5 shillings drawing on Lloyds Bank, uncancelled. 3 1/2x8 inches; faint scattered foxing, folds.
